FactorHealth has been providing pharmacy home care services to the bleeding disorders community since 2002.
The goal of FactorHealth's Hemophilia Health Management Program is to improve patient care while helping to lower costs to both the patient and the payers.
FactorHealth's Hemophilia Health Management Program encompasses the following features:
- Professional Standards: FactorHealth's professionals practice confidentiality consistent with standards of professional practice, and adhere to local, state and federal regulations at all times.
- Pharmacists and pharmacy home care services are available 24 hours per day 365 days per year. Our professional pharmacy team works with you and your patients on assay management, medication compliance, In-home inventory management assistance and tailored ancillary supplies to your needs.
- Pharmacists coordinate your patient's prescriptions with your office.
- Patients receive 24-hour home delivery as a standard.
- Emergency delivery in less than 24 hours when needed.
- Delivery to your patient's home, office or other location of choice.
- Our Patient Services Department will visit patients as they come on service and complete an Initial Assessment.
- As needed, FactorHealth's Service Team works with patients, their family and physicians to develop an Individual Care Plan.
- Follow up patient visits are completed annually or as desired thereafter. Assessments and care plans are updated annually or as required.
- Our Patient Care Coordinators maintain frequent contact with patients and your office as needed.
- Education- FactorHealth provides educational materials and programs for patients and other community members to help them become more self-sufficient in their care as well as become advocates for themselves and the bleeding disorders community.
