FactorHealth has designed a program to help support you in obtaining the products and services you need, when you need them. Our Team is dedicated to ensuring that you have the care, access and choices you require on issues that impact you and your family.

Team Approach

At FactorHealth we take a team approach to address each individual set of circumstances. Your Personal Services Team consists of a Benefits Coordinator, Patient Care Coordinator, Chronic Disease Services Specialist, Pharmacist and access to both a Clinical Social Worker and a Registered Nurse. FactorHealth works with you to create a program that fits your lifestyle.

Our services encompass the following features:

  • Pharmacists and pharmacy services are 24 hours per day 365 days per year, throughout the United States.
  • Professional pharmacy team works with you and your physician on medication compliance, ancillary supplies tailored to your needs, In-home inventory assistance management, assay management, mail back Sharps containers as needed and pharmaceutical counseling by registered pharmacists.
  • 24-hour home delivery as a standard.
  • Emergency delivery in less than 24 hours is available when needed. We deliver to your home, office or other location of choice.
  • Personal Patient Care Coordinator completes an Initial Assessment of your medical history and any other items pertinent to your history and current treatment regimen.
  • A Plan of Care which is a collaborative effort between you, your physician and Personal Services Team is created based on your individual and specific treatment goals and objectives.
  • Follow Up to assure that your needs are being met and you have access to the resources that you require to help you succeed including coordination of nursing, physical therapy, dental and nutritionist.
  • Community referrals as identified.
  • Patient Services Department provides education and the provision of educational materials to patients and family members.
  • Participation in all Pharmaceutical Assurance/Assistance Programs.
  • Professional Provider Directory that lists local and national community resources that are relevant to each community we service.
  • Medic-Alert-Information about enrollment in the Medic-Alert program is available upon request.
  • Patient Travel Packets which include a physician order sheet, travel letter and other items as required.
  • Member of the Patient Services Inc. Items Program (PSIp).
