All you need to do is ask your physician to call or fax us and we’ll take care of the rest!
Initial Prescriptions
- Ask your physician to FAX the prescription to 561-995-9162 or
- Ask you physician to PHONE in the prescription to 800-223-7151 or
- MAIL the prescription to:
