Living with Hepatitis C can be a challenge. At FactorHealth we take a team approach to address each individual set of circumstances. Your Personal Services Team consists of a Benefits Coordinator, Patient Care Coordinator, Chronic Disease Services Specialist, Pharmacist, and access to both a Clinical Social Worker and a Registered Nurse. FactorHealth works with you to create a program that fits your individual needs.

The Team Approach

Members of your Personal Service Team work with you to:

  • Manage your medications through our Pharmacists and pharmacy services that are available 24 hours per day 365 days per year, throughout the United States. Our professional pharmacy team works with you on medication compliance, ancillary supplies tailored to your needs and pharmaceutical counseling by registered pharmacists.
  • Complete an initial assessment of your medical history and current treatment regimen and create a plan of care based on your specific treatment goals. This plan is a collaborative effort between you, your physician and the Patient Services Team to develop successful strategies.
  • Communicate with you on a weekly basis for the first four weeks of your treatment and monthly thereafter assuring that your needs are being met and you have access to the resources that you require to help you succeed.
  • Provide the educational tools for good decision-making.
  • Access the Prescription Assistance Programs- FactorHealth works with all the major pharmaceutical companies and can assist you in signing up for these programs as needed.
  • Provide you with our toll free number to contact your Pharmacist and/or Patient Care Coordinator 24 hours a day/365 days a year.
  • Access Professional Services Coordination -nursing, physical therapy, dental, social services and nutritionist coordination when needed.
  • Utilize our Professional Provider Directory that lists local and national community resources that is relevant to the hepatitis C community.

Confidentiality

Patients Maintain Anonymity in their Community during Treatment

  • Medications shipped directly to patient via unmarked Fed-Ex.
  • Discreetly speak with a knowledgeable patient care coordinator or a pharmacist 24 hours a day, 365 days a year.
  • Mail back Sharps containers.
